Who Qualifies for Home Care?
Any person who is:
- under the regular care of a physician
- homebound in a safe environment
- requiring part time/intermittent skilled nursing and/or therapy services
Our care programs at Health Choice Home Care Service, LLC are delivered on an intermittent basis according to a plan of care that is established by the patient, family members, physician and the home health staff.
